This collaboration between TRACS WA, WACHS Chronic Conditions Strategy Team (CCST), Heart Foundation WA and the Australian Cardiovascular Health and Rehabilitation Association WA (ACRA-WA) have planned and developed a series of Cardiac Rehabilitation and Secondary Prevention (CRSP) telehealth professional development sessions for country WA Health professionals. Four successful monthly sessions have been delivered using video conferencing by highly experienced presenters on Thursday afternoons from August to November, engaging over 45 sites across WA.

Topics covered included:

‘An introduction to Cardiac Rehabilitation and Secondary Prevention (CRSP)’ - this training covered the basics of cardiac rehabilitation, adapting models to country settings

‘Exercise for Heart Health’ - this training covered chronic disease self-management, exercise plans and guidelines, monitoring blood pressure and heart rate, and managing medications

‘Supporting Your Patients – After Heart Attack’ and ‘Heart Failure and Arrhythmias’ - these sessions covered outlines of actions plans, information on medicines, stents, follow-up protocols using case studies, heart failure management and arrhythmia overview, patient journey, risk factors, follow up and action plans and the importance of explaining and demonstrating diagnosis to patients.
