Staff on Ward V5 (rehabilitation) at Fremantle Hospital completed a TRACS WA Workplace Learning funded activity to improve the quality of goal setting practice on the ward.  The project, initiated in 2017 by Kathryn Ramsay, Senior Speech Pathologist was taken over by Tanya Regan, Senior Physiotherapist and the team at V5. The overall project goal was to embed client-centred, values-based goal setting processes across all medical teams inclusive of medical, nursing and allied health staff on Ward V5.

The project was multifaceted and included:
  • a review of literature on goal setting
  • an audit of current processes to incorporate goals into the MDT meetings
  • development of a goal setting orientation package for new staff, including a competency check list
  • an electronic form to record goal setting meetings, mentoring of staff and targeted education and evaluation of utilisation of the electronic goal setting meeting record
