Setting goals is important for rehabilitation and patient-centred care. Having discussions with people who have a life limiting illness about Goals of Patient Care is also important during a hospital admission, to enable respect for patient choice and a dignified end of life.
This 30 minute toolbox recorded session is a must for anyone working with people with complex medical conditions, elderly patients, or those with a life limiting illness. It provides an introduction to the Goals of Patient Care document, and how it fits within Advance Care Planning. It covers communication strategies to help clinicians hold supportive discussions around end of life preferences. This toolbox also contains links to an extensive list of resources for further information, videos and online training from leaders in palliative care and research around Australia.
