The NDIS-Health Community of Practice is co-hosted by TRACS WA and the Disability Health Network. We have run a number of events for clinicians, mostly working in the public health system. The long term goal is to develop a self-sustaining CoP for health professionals who interact with the NDIS, or who treat clients who do so, to facilitate networking, sharing of experience/resources, and problem solving to provide the best possible service for people with long term disability who are funded by the NDIS, but who also access mainstream health services.
Topics for this session will include:
• The new Exceptionally Complex Support Needs Program
• NDIS Hospital Liaison Officer roles
• NDIS linkage project being undertaken at FSFHG
Time will also be allocated to ‘going round the room’ to allow participants to highlight an NDIS-Health related issue in their area, or a project or innovation that might be of interest to their colleagues in other health services. We hope to see you there.
