On Thursday 25th March, in collaboration with Disability Health Network, TRACS WA co-hosted our 8th CoP meeting. This focussed on enabling discharge from hospital for NDIS participants.
There were three presentations: Jenny O’Callaghan (Clinical Lead NDIS/WA Health Interface, Patient Safety and Clinical Quality, DOH) provided an update on the NDIS Hospital Liaison Officer role.
Danielle Loizou-Lake and Jen Hart (Project Leads Disability Health Projects, National Disability Services) summarised a very comprehensive review conducted as part of the Ready to Go Home project – available on request – and gave an overview of the project. The need for extensive interagency collaboration was highlighted by all these presenters.
Finally, Kylie Mucciarone, (NDIS participant, MHRM) provided some great insights into the challenges of the discharge process and timeliness of post hospital support and equipment for those with a physical disability.
Over 40 people attended from a range of services. We aim to host at least two more NDIS-Health CoP meetings in 2021. Please get in touch if you have a particular topic that you would like to see addressed.
